Review process for the DDLA form

The "School" has been designated as the reviewer on the current DDLA form template in Dynamic Forms. This introduces several important changes to the process for reviewing the form:

  • The originator of the form is no longer required to specify the reviewer.
  • The reviewer is the last participant in the document work flow and will review the form after all participants in the work flow have signed the form.
  • The reviewer is not required to sign the form, rather they will "complete the form" and move it directly to the 'Processed' queue upon completion of their review.
  • The reviewer will still be able to return the document to any of the participants if there is an issue with the completed form.
  • Should a reviewer accidentally "complete" a form, the KSU - DDLA org admin has the ability to unprocess the form.

NOTE: Dynamic Forms does not support conditional rules/logic to send specific reviewers an individual email notification based on the originator/college/school/department. A single email notification will be sent to ALL designated reviewers (see the table below for a list of reviewers.)
 

Reviewing the DDLA Form

The Admin email notification in the DDLA form settings has been updated to include information in both the Subject line of the email and the Body of the message to include the name of the ORIGINATOR of the form.

Upon receiving a notification for a completed DDLA form, make sure to check the originator of the form. Review only forms originated by the designated staff in your college, otherwise ignore and delete the email notification.

Completing the DDLA Form

The email notification sent to designed college reviewers contains instructions for "completing" the form and a link to log into Dynamic Forms.

Note: "Complete form" is Dynamic Forms' terminology for the process used when the School has been designated to review and move a form from the Multi Pending queue to processed. The DDLA form document has already been completed, i.e., signed by all required parties. The reviewer is not required to sign the form. If the form is never "completed" (reviewed and moved to processed) in Dynamic Forms, the DDLA is still a signed and binding agreement.

Log into Dynamic Forms, then click on the circle icon in the Multi queue for the Distance / Distributed Learning Agreement form.

If there are multiple forms in the Multi Pending queue, sort the forms by the ESigned By column to locate the forms initiated by your college/academic unit. To review and complete the form, click on the Complete Form action listed in the School Action column.

Click on the Next button to navigate through the pages of the signed DDLA document. If the form requires revisions, click the Return for Revision button.

Note: Clicking on the Next button on the LAST page of the document will automatically "complete" the form and move it from the Multi column to the Processed column.
